Addressing Social Determinants of Health: Maureen’s Story
Boston Medical Center’s THRIVE Social Determinants of Health Screening & Referral Program is expanding care beyond the realm of traditional medicine by looking at all of the ways a patient’s health might be affected. As part of the program, patients are given a brief survey which screens for issues related to social determinants of health, like housing and food. When Maureen filled this out, it revealed she had lost her job and was struggling to find stable housing. Her doctor and care team at BMC were able to connect her with key resources to address the challenges she was facing. Today, she has a home to call her own.
