Addressing Social Determinants of Health: Celia’s Story
At age 78, Celia became homeless. But thanks to Boston Medical Center’s Elders Living at Home Program, that is no longer the case. Addressing social determinants of health—like homelessness and housing insecurity—is just one way BMC is working toward making Boston the healthiest urban population by 2030, as part of its Vision 2030 initiative.
